Health Screening
For regular employees with existing district Health Benefit Plan
Your numbers tell a story. What do they say about your health? Find out, earn cash and get healthy. Get your numbers for free at the LAST health screening event for employees to be held at Quartz Hill High School on 4/13/10.
Appointments book quickly, don't delay! Each screening takes about 10 minutes and includes:
Total cholesterol and HDL
Blood pressure and pulse
Blood glucose
BMI (Body Mass Index) and other key ratios
To participate in the health screening event, simply schedule an appointment by clicking on this link http://sisc.kern.org/healthsmarts When you first visit the website, register as a new member. Then select our district. Next, click on a time for your appointment and follow the prompts to complete the process. You will receive a confirmation number and a confirmation email - bring these with you.
All enrollees will be directed to the same website and the eligibility in the system will direct them to the correct place. Spouses and adult dependents are eligible for the awards only if they are enrolled on the health plans (PPO, HMO or Kaiser).
